How Can I Access My Sick Note/Referral/Lab Requisition?

How Can I Access My Sick Note/Referral/Lab Requisition? Log in to theqdoc.com. Scroll down to your patient profile. Click “View Documents”. QDoc will send a 6-digit confirmation code to your phone – please enter it and click “Verify”. When in “View Documents”, you will be able to see the name of the QDoctor and the type of document – click the “Preview” (?) button to view and download documents. If you are using a smartphone, you may have to scroll to the side to see the button. How QDoc works

How QDoc works QDoc is a tool physicians can use to work when they want, where they want. See patients conveniently from the comfort of your own home. QDoc focuses on ease of use and convenience for both doctor and patient. Governments and insurance companies are billed directly, you are notified by text when there is a patient to be seen, administration needs are handled, along with many more exciting features. When there are patients to be seen, you will be notified by text. Click on the text if you want to, otherwise, another QDoc doctor will pick up the patient. If you are ready to see a patient, click “accept patient visit” and start an end to end encrypted video call with the patient. A transcript of the visit is attached to the patient’s chart using voice recognition as well as an audio file. If the patient requires additional care, you may direct patients to be seen physically at either one of our partner clinics or your own clinic(s). How Can I Get My Lab Test Results?

How can I get my lab test results? If you have not heard from a QDoctor by two days after your lab tests, please log on and start a visit to get a QDoctor to go through your results with you. After they have been reviewed by a QDoctor, results will be accessible on your patient card: Click “View Results” on your patient card. QDoc will send a 6-digit confirmation code to your phone – please enter it and click “Verify”. When in “View Results”, you will be able to see the name of the QDoctor who ordered the test and the type of document. Click the “Preview” button to view and download your lab test result. If you are using a smartphone, you may need to scroll to the side to see the button. How Can I Access My prescription?

How can I access my prescription? During your visit, the doctor will ask for your preferred pharmacy, where your prescription will be sent.  Please contact the pharmacy to confirm that the prescription is ready for pick-up. If you can no longer access that pharmacy, please call the QDoc help line at 1 (833) 736-2362 and specify where you would like the prescription to be redirected. How Do I Change My Patient Info?

How do I change my patient info? Go to theqdoc.com and log in. Scroll down to the bottom of the page to your Patient Profile. Hover over your patient card and click on the pencil icon at the upper right corner. This is located right under the patient’s name. QDoc will send a code to your phone to verify your account. If you do not have access to the phone number listed on your profile, select SMS and change the code delivery method to Email. Click “Resend”. Enter the code and press “Verify”. Edit your information as necessary and click “Save”. Can I get lab tests or a referral? Yes – during your visit with a QDoctor, the doctor can order lab tests or write a referral for you to see a specialist if they feel that it is necessary and appropriate. You will be able to view, download, and print your referrals and lab requisitions by clicking the “View Documents” button on your patient card. Can I get a doctor’s note?

Can I get a doctor’s note? Yes – if you request a sick note during your visit with a QDoctor, the doctor can complete and sign a note that you can then access by clicking the “View Documents” button on your patient card. For more information on how to access your sick note, view the section ‘How can I access my sick note/referral/lab requisition?’. A fee of $30.00 may be applicable for the issuance of sick notes. Will QDoc share my visit information with my primary care doctor?

Will QDoc share my visit information with my primary care doctor? Immediately following your visit with a QDoctor, you will be asked whether you would like a copy of your Encounter Summary to be sent to your primary care doctor.  If you select “yes,” then a summary of your visit will be automatically sent to your primary care doctor, as listed on your patient profile. If you select “no,” then your information will not be sent anywhere, and a copy of your Encounter Summary will be saved to your QDoc patient profile.
