How to Get Referred

Start a visit

Join the queue on QDoc by selecting 'mental health' and 'ADHD Screening' as your reason for visit.

Fill out forms

Fill out the necessary pre-visit and pre-screening forms.

Request a referral

Start your visit with a provider. If appropriate, the provider may refer you to our Adult ADHD Clinic or Children’s ADHD Clinic.

Begin referral process

The referral will be sent and you will be notified on the status and booking of your consultation appointment.

This clinic is meant for patients who:

Have been previously diagnosed and are seeking ongoing care or treatment optimization.

Are experiencing symptoms of ADHD and want a professional assessment.

Are over the age of 6 years old.
